TO CANCEL OR RESCHEDULE AN APPOINTMENT

If you would like to reschedule your appointment you can call the North Carolina Children’s Hospital Pediatric Clinic at 919-966-1401. If your appointment was at the Prestwick building, you can call 919-966-4401. If your appointment was at the Lumberton Children's clinic, please call Susan at the clinic at 910-739-3318.

FOR NEW PATIENTS

If you are new to UNC Child Neurology, please be sure you come to your visit with your records including any records from another child neurologist. If you have had MRI or CT scans of the brain or spine, please try and get copies of the studies on a CD/DVD and bring them to the visit. You can usually call the facility where the study was done and they will be able to get a copy. Often they will mail it free of charge to us although sometimes they will want a small charge if they burn a disk for you as the patient.

FOR ALL PATIENTS

For all patients we encourage you to bring all of your medicines in their pharmacy labeled bottles so we can be sure about your medicines and dosages.

IMPORTANT – PATIENT QUESTIONNAIRES

We request that all patients, new and returning, go to our patient intake website to securely fill out our patient questionnaires before your visit. We have found that the answers to the extensive questions we ask give us a very good picture of your medical condition and allows us to track your progress carefully over time. By filling out these forms from home you can give us better information by taking your time and the important happy consequence is that it will speed up your clinic visit. If you are not able to fill out the forms for some reason we will try and gather the information when you arrive in clinic.
