Is this your first login for an upcoming visit with UNC Child Neurology? If yes, login with the patient's last and first names and date of birth.

After entering your login information you will be taken to a screen with a passcode. Write this code down and keep it in a safe place.

If you start but do not complete the forms and you log out and come back later to finish your work you will need the passcode in addition to the child's name and date of birth to be allowed back into the system. This is done to securely protect your data. You will get a different passcode for each Neurology visit so the passcode you receive today will not work for any other appointment except the upcoming one. If you complete the forms in one sitting, you will not need the passcode.

If more than one child is being seen, you will need to log in and obtain a passcode for each child individually.

If you forget or lose your passcode, you can call the office at 919-966-2528 and the secretary can retrieve it for you.
